Translation of the title statement: Sefer Toledot Adam, authored by Rabbi Elijah Ba'al Shem, and Rabbi Nathan David from the Tismenitz community (Tysmenytsya, Galicia) rose to (work toward) bringing as the second time to the printing house, and incidentally, he printed also Sefer Segulot ve-Refuot, (remedies by) herbs and spices, from the advice of expert physicians ... also with this some Segulot (rituals) and (religious) Remedies, from the Sefer Refuot that was printed in Jessinitz in Ashkenaz language (yiddish-daitsh) excerpted by Rabbi Nathan Pless. On the attribution of this volume: Friedberg presumes the attribution to Elijah Ba'al Shem without ennumerating, but this work is really attributed to Loanz (and not Ba'al Shem of Chelm as it appears). Steinschneider lacks this imprint but mentions the first printing (sans Pless) in Cat.Bodl. 5833:(1), "Medicanta et Specifica; in lib. anon. Toledot Adam". Lewenstein (נפשות צדיקים - Epitaphien von Grabsteinen, Frankfurt am Mein 1855) p.59 ascribes this Ba'al Shem as an author of Kabbalistic books and works of amulets and holy letters, although he doesn't mention this work. Loanz (not Elijah of Chelm, as a great confusion has occured many times between the two, con. cit. Heilpren, Seder Ha-dorot 333, and S.J.Feunn, ḳiriyah ne'emanah pp.58) was documented as a worker of Kaballah ha-ma'asit, "creative Kabbalah", and not speculative Kabbalah. I bring in the note of Dembitzer, Kelilat Yofi (ḳraḳow 1888) p.172 about Elijah ba'al Shem of Chelm, ancestor of Zevi Ashkenazi, not writing and not dominating as a practitioner of Kabbalah ha-ma'asit (aside from one incident mentioned in Shu.t. ḥakham zvi 33 and Sh.u.t. Sha'ar Efraim). - Chaim Meiselman- Place
