- This is what I could read from the inscription: אני מעיד על שהספר יפה מראה שייך הגאון החסיד המפורסם ומורי רב כ"ה שמואל פ"פ ? ? ? א ח תמוז תקוח

- It seems like the date that I could decipher is ח תמוז תקוח but I am not sure that is correct, as I don't know why the year would have been written that way (and not תקיד). And perhaps the פ"פ after "שמואל" is a reference to Frankfurt? There is other evidence of the book being in Frankfurt; see other footprints for this copy. Can you help?
